Fees

Individual therapy $180 

Sessions are 50 minutes

Acceptable Forms of Payment

All major debit and credit cards, Health Savings Account (HSA) and Flexible Savings Account (FSA), cash, check.  

Insurance

am not in-network with any insurance companies and do not bill insurance directly. With PPO plans, I am considered an out-of-network provider and depending on your specific health plan, you might be eligible for partial reimbursement. As a service to you, I can provide you with a monthly invoice (Superbill) that you can submit to your insurer for possible reimbursement.  It is best to contact your insurance provider prior to starting therapy to verify what may or may not be covered by your plan and whether you have a deductible to meet prior to any reimbursement being paid to you.  Payment is expected in full at the beginning of each session.

Employee Assistance Programs (EAP)

I'm a contracted provider with Lyra Health and Modern Health, which are only available through select employers.  You can ask your Human Resources Department if your employer participates with one of these EAPs.

Questions to ask your insurance company:

1. Do I have Out of Network behavioral health benefits? 

2. Do I have an Out of Network deductible, how much is it and has it been met?

3. What is my Out of Network reimbursement rate per session?

4. Are there limitations based upon the length of session (45 minutes versus 60 minutes)?

5. How many sessions are covered per calendar year?

 

 

Good Faith Estimate
